The Adult ADHD Self-Report Scale (ASRS) is a widely used, 18-item questionnaire developed by the World Health Organization (WHO) to screen for and monitor Attention-Deficit/Hyperactivity Disorder (ADHD) in adults.

Please be advised that the results of this checklist are not confirmation of an official diagnosis. It is simply a tool to gage a potential likelihood. To begin the process to receive a proper diagnosis, talk with your primary care provider.
